Got something to tell us?
There are a number of ways you can get involved in surveys. You may be asked to give your feedback through a survey or it may be sent to your home address (with your consent). Alternatively there may be online surveys on this website in which you can take part.
You may also be invited to take part in projects where your opinions and views are of vital importance.
Your feedback is important to us and will help us to provide a quality service which meets the needs of our patients. We would therefore be grateful if you could find a minute or two to fill in these quick patient surveys.

Annual member and patient survey – listening to your views
Each year we undertake a membership and patient satisfaction survey to give members of our foundation trust and service users the opportunity to comment on the services and activities we provide and the extent to which these meet and reflect their needs. These surveys are important to us, giving us useful information on where our focus should be as well as helping to shape our thinking as we draw together our future plans.
