Make a Solicitor’s Request
Solicitors can make enquiries and apply for health and call records, including patient report forms, under the Data Protection Act 1998 and Access to Health Records Act 1990 in connection with a civil claim or criminal matter. Any charges for this information will be confirmed at the time the request is received.
Submit a Freedom of Information request
You can make a formal request for information under the Freedom of Information Act by contacting our Information Governance Manager.
Our Freedom of Information publication scheme has been produced in line with section 20 of the Freedom of Information Act 2000, under which the Information Commissioner has approved a new model publication scheme for use by all public authorities from 1 January 2009.
Make a request relating to your records held by the Trust
Under the Data Protection Act 1998, you are entitled to access the data processed by the Trust which is personal to you. This may include copies of records held by us. A charge may be made for supplying this information.
