Got something to tell us?
There are a number of ways you can get involved in surveys. You may be asked to give your feedback through a survey or it may be sent to your home address (with your consent). Alternatively there may be online surveys on this website in which you can take part.
You may also be invited to take part in projects where your opinions and views are of vital importance.
Your feedback is important to us and will help us to provide a quality service which meets the needs of our patients. We would therefore be grateful if you could find a minute or two to fill in these quick patient surveys.

Have you used any of our main services (999, NHS 111 and Non-Emergency Patient Transport) in the last year? Our Member Satisfaction and Patient Care 2017 Survey is now open and it’s your chance to give us your feedback and help us shape the Trust’s plans for the year ahead.

If you have not used any of our services but are one of our 13,500 foundation trust public members, you can still take part and just complete the first section of the survey. Your voice counts!
